Just did an install of fop2 on a PIAF system. I am new to PIAF so have probably done something wrong, but I have the following:

After installing FOP2 2.22 I installed "FOP2Admin FreePBX Module v 1.1" (marked OLD) as v 1.2 was not listed.

I now have three problems - maybe connected.

1. My FOP2 screen is just blank - no buttons at all
2. My FOP2 screen seems to have the left edge chopped off: of the text "Filter" only "lter" can be seen.
3. /usr/local/fop2/fop2_server --test

returns:

Flash Operator Panel 2 - No valid license found. Demo Mode
ERROR 1146 (42S02) at line 1: Table 'asterisk.fop2settings' doesn't exist
ERROR 1146 (42S02) at line 1: Table 'asterisk.fop2permissions' doesn't exist
ERROR 1054 (42S22) at line 1: Unknown column 'channel' in 'field list'
Connection to manager OK!

Any help would be much appreciated.
